(4) GENERAL TERMS & CONDITIONS
- Persons between the ages of 18 to 65 can apply. Children from the age of 3 to 18 (23 if studying) can be covered however Benefits 1, 6, 9 and 10 are excluded. Children are entitled to only 50% of the other Benefits.
- The premium rates stated are based on the assumption of normal occupational exposures and normal physical health.
- The company reserves the right to review each proposal and premium rates will not be in force until the application has been accepted by us. Applicants can only select one (1) of the four (4) plans.
